New Patient Registration

The practice list is currently open and we are happy to accept new patients who live within our  boundary area (This includes Hartlepool and also the surrounding villages or Hart, Elwick, Dalton Piercy and Greatham, but excludes the Headland). Patients wishing to register with the practice will asked to complete a registration application form which can be obtained from reception.

As part of the registration process you will need a 20 minutes appointment with the Health Care Assistant for a simple check up and to fill in a questionnaire giving details of any past medical history. This helps us to provide you with an efficient service and allows us to collect necessary details about you and your health pending receipt of your full medical records (this can take several weeks)

Failure to attend for a booked appointment without cancellation may mean the practice will not accept your request to register nor will you be able to attend on an emergency or temporary basis. 

The practice may have periods of time when it does not accept new registrations wherever they live.
